The good faith exam is a brief virtual consultation with our collaborating physician. It is an essential step in ensuring we understand your health needs and can provide the safest, most personalized care tailored specifically to you.

During this consultation, we review your medical history, current medications, and any health concerns to determine if IV therapy is right for you.

Payment: No upfront payment required. Payment is collected when you come in for your first service.

HOW TO COMPLETE THE GOOD FAITH EXAM

Option 1: Complete Ahead of Time
RECOMMENDED

To streamline your visit, we encourage you to complete the good faith exam and your intake/consent forms before you arrive:

  2. Answer questions about your medical history and current health

  3. Click the link below to complete and sign your intake/consent forms

  4. You’re done! Takes about 20 minutes

 

This process is quick and straightforward, allowing you to complete it at your convenience and in the privacy of our own home.

Option 2: Complete With Us In-Office (Tablet Assistance)

If you’d rather not complete the good faith exam and forms on your own, you can do everything in-office using our tablet.

I’ll walk you through the process step-by-step and help with any questions or technical support along the way—so you feel comfortable and confident before your first visit.

Note: If you complete your good faith exam and paperwork in-office, your appointment will take longer—plan for about 75–90 minutes total (paperwork + infusion). If you complete everything ahead of time, most infusion appointments are about 45–60 minutes. If you’re only getting an IM shot, you can typically be in and out in about 5–10 minutes, as long as your exam and paperwork are completed ahead of time.

WHAT HAPPENS AFTER THE EXAM?

Approved:
Our physician reviews your information and approves you for IV therapy. You’ll receive confirmation and can schedule your first appointment.

Not Approved:
In rare cases, certain medical conditions may make IV therapy inappropriate. We’ll discuss this with you and recommend you speak with your primary care physician about alternatives.

CONTRAINDICATIONS FOR TREATMENT

The following conditions may result in denial for IV therapy:

  • Congestive Heart Failure

  • Pulmonary Edema

  • COPD or Emphysema

  • Liver Failure

  • Renal Failure or Chronic Kidney Disease

  • Anasarca or Ascites

  • Tachyarrhythmia

  • Active chest pain, shortness of breath, or acute status change

  • Allergy to cobalt, corn (Vitamin C), or sulfa*

 

*Sulfa allergy is not a strict contraindication but requires caution with Glutathione due to detox sensitivity.

 

Pregnancy: Approved up to 13 weeks gestation; after that, IV therapy is not recommended.
