9. Service Terms

Telehealth Consultations:

A telehealth consultation with our collaborating physician is required before your first treatment
Telehealth consultations are non-refundable
You must be in Ohio to receive services
You acknowledge that telehealth is not a substitute for in-person medical evaluation

Treatment Services:

All treatments are provided by licensed registered nurses following strict medical protocols
You are responsible for disclosing your complete medical history and current medications
You agree to inform our staff of any allergies or sensitivities before treatment
You assume all risks associated with IV therapy and injections

Memberships are month-to-month with no lock-in period and will automatically renew each month unless you cancel before your renewal date to avoid being charged for the following month.
No refunds are issued for unused services.
Unused infusions do not roll over to the next month.
Annual telehealth exams are required to maintain membership eligibility.

Cancellations and Rescheduling:

Contact us as soon as possible to reschedule or cancel appointments
We will do our best to accommodate rescheduling requests
Cancellations made less than 24 hours before your appointment may result in a cancellation fee

Payment:

Payment is due at the time of service unless you have an active membership
We accept credit cards, HSA/FSA cards, and cash (for non-membership services)
A 3% processing fee applies to card payments
We do not accept insurance at this time

10. Assumption of Risk

You acknowledge that IV therapy and injections carry inherent risks, including but not limited to: infection, bruising, nerve damage, allergic reactions, and adverse effects. All treatments are administered by licensed, experienced registered nurses following strict medical protocols and under physician oversight. You assume all risks associated with receiving these treatments and agree to hold harmless Retreat Mobile IV Hydration LLC, its staff, and its collaborating physician from any claims or damages arising from your use of our services.

11. Medical Disclaimer

The information provided on our website and during consultations is for educational purposes only and is not a substitute for professional medical advice. Always consult with your primary care physician before starting any new treatment. Our services are provided by licensed healthcare professionals and are subject to medical oversight by our collaborating physician.
